How you’ll make an impact in this role
- Continuum of Care: You are the "architect" of the patient’s journey, ensuring their medical, social, and spiritual needs are met from the moment they are admitted until they safely transition home or to another facility.
- Clinical Advocacy: You identify barriers—such as lack of insurance, transportation, or home support—and find creative ways to remove them to prevent readmissions.
- Documentation & Compliance: You serve as a bridge between the clinical team and payers, ensuring medical records accurately reflect the level of care required for reimbursement.
- Mission-Driven Service: Unlike purely administrative roles, this position emphasizes Reverence and Integrity, treating discharge planning as a "ministry" of hope and dignity for vulnerable populations.
Minimum Requirements
- Licensure / Certification / Registration: Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
- Certified Case Manager: credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
- Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
