Case Management Manager

Responsibilities

• Assisting the case management director with hospital case management
• Assisting the case management director with social work
• Assisting the case management director with transition management
• Assisting the case management director with operational management of the case management department

Qualifications

Required:

• RN
• Associate's degree in nursing or higher
• Social Work degree, BSW or higher
• 1+ years’ experience in a case management leadership role

Preferred:

• 2+ years’ experience in acute hospital-based case management
• Certification in case management (ACM or CCM)

Pay

Rate of pay is determined based on experience and education and may include other pay components such as differentials and call pay based on role.
