Introduction

The Case Management Manager shares responsibility for the planning, direction, coordination, and control of activities concerned with case management. Assists in the provision of resources and assists to remove barriers to successful operation of case management.

Job Summary and Qualifications

Summary: The Case Management Manager shares responsibility for the planning, direction, coordination, and control of activities concerned with case management. Assists in the provision of resources and assists to remove barriers to successful operation of case management.

Position Requirements

  • Licensure/Certification/Registration: Current licensure in the state of Colorado as a Registered Nurse or Licensed Clinical Social Worker (LCSW). Current BLS certification through the American Heart Association or American Red Cross is also required.
  • Education: BSN required if Nurse, or Master’s required of LCSW
  • Experience: At least three of case management experience required. At least two years previous experience in charge/supervisory or management position preferred. Able to demonstrate leadership and management skills.
  • Special Qualifications: Must have the ability to assess information, apply critical thinking process to determine patient's progress. Requires excellent human relations and oral/written communication skills. Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations and team collaboration and patient interviews along with customer service skills. Must also have a strong analytical and organizational skills.
