Case Management Coordinator - Psych Acute

Responsibilities
- Coordinates and reviews flow of files and documentation in the Department.
- Actively participates in Case Management and Treatment Team meetings.
- Reviews patient charts to assess whether criteria for admission and continuation of treatment have been documented.
- Gathers data and responds to requests for records from fiscal intermediaries.
- Gathers clinical and fiscal information and communicates the status of both open and closed accounts for multiple levels of Case Management reporting.
- Works independently and uses sound judgment.
- Applies knowledge of Federal, State, and intermediary guidelines related to inpatient, acute care hospitalization, as well as lower levels of care.
- Coordinates communication regarding discharge referrals as requested by clinical staff, fiscal intermediaries, patients, and families.
Requirements
- Associate or Bachelor’s degree in healthcare or a related field, or equivalent work experience in the related field.
- Acute care work experience preferred.
- Intermediate computer skills, including data entry.
- Proficient communication skills, both written and oral.
