Key Responsibilities
- Provide comprehensive case management using person-centered approaches that fully include youth in decision-making, daily routines, and goal planning while promoting developmental/academic growth, independence and cultural relevance.
- Lead, schedule, and participate in coordinated service planning meetings with team members and external providers.
- Assess the effectiveness of services and interventions delivered within the home, addressing gaps and adjusting plans as needed.
- Maintain accurate documentation and communication regarding youth progress, service needs, and interdisciplinary coordination.
- Participate in rotating on-call responsibilities to support program continuity and emergency response.
- Maintain the staffing schedule for the Horizons program homes ensuring that the homes have qualified staff working in the homes at all times.
Required Qualifications
- Candidates must meet all regulatory criteria for Case Management Supervisors, including:
- Minimum age of 25 years.
- Master's degree in social work or human/social services.
- At least 1 year of experience in child welfare or a human/social services setting.
- At least 2 years of experience in a residential childcare setting.
- Current Qualified Intellectual Disability Professional (QIDP) certification.
