About the role
The Case Management Assistant provides daily support to the Clinical Resource Management Department, facilitating quality and utilization data essential for day-to-day operations. This role ensures appropriate follow-up in administering Centers for Medicare & Medicaid Services (CMS) forms to enhance patient service quality across the system.
Responsibilities
- Provides CMS forms (MOONS, IMs) to patients in a safe, timely, and effective manner.
- Verifies and ensures accurate and confidential documentation in patients' medical records.
- Responds to inquiries from CRM leadership and responsible parties.
- Interacts with patients to track progress and ensure satisfaction.
- Adheres to professional standards as outlined by protocols, rules, and regulations.
- Develops effective working relationships with the CRM team.
- Facilitates multiple care aspects as requested by CRM leadership for discharge planning, including case coordination and information sharing.
- Assists with daily referrals, per standard protocols, to Primary Care Physicians and JHS specialty clinics, scheduling diagnostic studies, and other appointments.
- Coordinates with outpatient studies as needed.

- Performs all other related job duties as assigned.
Requirements
- Generally requires 1 to 3 years of related experience.
- High school diploma is required.
- Valid license or certification as needed, based on the job or specialty.

Working Conditions
Physical Requirements
- Job function is sedentary, requiring sitting for extended periods, with frequent standing or walking.
- Must be able to lift or carry objects weighing up to 20 pounds.
- Requires close visual acuity for extended computer use, data analysis, and other office activities.
Environmental Conditions
- Fast-paced environment with occasional high pressure or emergent and stressful situations.
- Frequent interaction with a diverse population, including team members, providers, patients, insurance companies, and the public.
- Subject to inside environmental conditions, with occasional outdoor exposures.
- Possible exposure to communicable diseases, toxic substances, medicinal preparations, and other hospital or medical office conditions.
- May require wearing Personal Protective Equipment (PPE) such as gloves or a mask when outside the office environment.
Schedule
Full-Time, Day Shift - Monday through Friday, 8:00 AM - 4:30 PM.
