Case Management Analyst - Field in LaSalle & Bureau Counties, IL
Position Summary
Counsels and educates members through the use of care management resources to ensure appropriate healthcare outcomes.
Required Qualifications
- Minimum 2 years of case management experience
- Reliable transportation and willingness to travel up to 75% of the time
- Mileage is reimbursed per company expense reimbursement policy
Preferred Qualifications
- Case management and discharge planning experience
- Managed Care experience
- Microsoft Office experience
Education
Bachelor's Degree or Non-Licensed Masters Degree in a behavioral health or human services field (psychology, social work, counseling and/or marriage & family therapy)
Pay Range
The Typical Pay Range For This Role Is $21.10 - $44.99

This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
