Case Coordinator

About the role

The Case Coordinator oversees psychiatric and medical treatment regimens for residents and provides case management services based on individual assessments.

Responsibilities

• Maintain case files and service plans for each resident
• Oversight of psychiatric and medical treatment regimens
• Conduct individualized assessments of residents' needs

Requirements

• Bachelor’s degree in the Human Services field
• Related experience and/or training in mental health field
• Experience with developmentally delayed, geriatric, and co-occurring disorder clients
• Valid driver’s license and acceptable driving record

$22-$24/hr
