About St. John’s

Founded in 1826 as a home for orphaned children, St. John’s has evolved into a nonprofit social services agency providing residential care to New York City youth involved in the child welfare and juvenile justice systems. We offer holistic, trauma-informed support to young people up to age 21. Our main campus in Rockaway Park, Queens, houses a Residential Treatment Center, Close to Home Non-Secure Placement programs, administrative offices, clinical services, and an on-site NYC Department of Education school. We also operate a Non-Secure Detention residence in Richmond Hill and Limited Secure Placement programs in Brooklyn and Staten Island. St. John’s is dedicated to helping youth heal, grow, and build the skills they need to achieve their goals and return to family or a stable, supportive environment as engaged members of their communities.

Responsibilities

  • Works in partnership with social service and residential departments as an integral member of the treatment team to provide direct services to youth and their families.
  • Follows program’s Evidence Based Model in developing and executing a permanency plan for youth.
  • Schedules and assists with coordination of parent/child and/or sibling visitation schedules and ensures follow through/compliance.
  • Submits to team weekly; coordinates with Case Planner and develops monthly treatment plan conferences.
  • Transports and attends court proceedings, medical, clinical, educational, vocational, housing and other appointments on behalf of youth, with youth and program staff; advocates for youth as appropriate.
  • Attends and actively participates in treatment team meetings, release/discharge conferences, Red Flag meetings, Family Team Conferences, Transitional Meetings, IEP conferences and other meetings related to the resident plan.
  • Meets with youth, as needed, to discuss safety, well-being, permanency, and transitional progress; provides crisis intervention and task-centered treatment.
  • Enters progress notes inclusive of all face-to-face contact/collateral and relevant case information regarding youth into Connections database.
  • Meets regularly with supervisor for supervision to discuss case/task-related challenges, progress and ideas.
  • Attends scheduled unit meetings with Director of Social Service, caseworker and other support staff to discuss challenging cases, exchange treatment ideas and receive updates regarding agency, ACS and OCFS policies for compliance.
  • Performs administrative duties including input and tracking of incident reports, visitation, resident placement disruptions (MYEVOLVE), resident restraints (ESPIS) into ARTS system (OCFS system)/Connections, and AWOC and critical incident documentation.
  • Attends school conferences and IEP meetings; observes youth’s behavior and academic progress during weekly school visits; meets with youth to discuss observations, reports and offers support, guidance and suggestions.
  • Engages with parents during home assessments, treatment team and release/discharge conferences; assists in obtaining medical consents and vital documents from parents; provides support and technical assistance to parents as needed.
  • Attends required/offered training sessions at ACS, OCFS and agency.
  • Provides temporary coverage of uncovered cases as needed.

Minimum Requirements

  • Bachelor’s degree in Human Services or related field preferred – OR – an Associate’s degree in Human Services-related field.
  • Valid NYS Driver’s license in good standing.
  • Knowledge of child welfare system preferred, specifically in congregate care setting.

  • Strong organizational, presentation and writing skills.
  • Works until the job is complete; able to think/work independently, manage crisis/conflict.
  • Works effectively as a member of the team.
  • Ability to effectively engage resident youth despite possible behavioral/cognitive challenges.
  • Fidelity to Missouri Approach/program-specific therapeutic intervention.
