Responsibilities and Duties
- Provide care to adults with a diverse range of issues, such as substance use, functional impairments and psychosocial needs (homelessness, justice involvement and unemployment)
- Aid in implementing the established program policies, mission and procedures as they relate to the program.
- Maintain a caseload and provide substance use assessments, counseling, referrals, intervention services and perform scheduled tasks as assigned.
- Intervene with patients regarding special problems, as needed.
- Aid in facilitating intake and orientation of new patients.
- Maintain accurate up-to-date clinical record keeping in compliance with regulations.
- Contribute input for Performance Improvement with regard to quality of care and planning clinical services
- Effectively provide for crisis intervention with patients and/or family
- Aid in developing and implementing program goals.
- Aid in outreach to other agencies/communities and outreach absent patients.
- Aid in coordinating services with relevant service agencies for individual patients; refer patients to other programs as needed.
- Follow up with psychiatrists and/or attending physicians when relevant and appropriate.
- Utilize validated tools to screen and assess patients for depression, trauma, substance use, risk and needs associated with recovery and relapse prevention.
- Evaluate, on a continuing basis, use of services and expected outcomes for patients.
- Provide culturally competent services in accordance with Hands On Health policies and procedures.
- Develop group curriculum and facilitate or co-facilitate group psychotherapy.
- Develop recovery-oriented treatment plans and reviews in collaboration with clients and consented stakeholders such as significant others, criminal justice personnel, primary health care providers and other service coordinating agencies.
