About the role

The HRA Office of Special Services administers specialized programs for eligible individuals in various settings to promote independence and well-being. The Home Care Services Program (HCSP) is a Medicaid-funded program that provides long-term care to medically fragile and vulnerable individuals through CASA, MLTC Program, Assisted Living Program, and Homebound Medicaid.

Responsibilities

  • Supervise case management teams ensuring appropriate and timely determinations are initiated through the Long-Term Care Web.
  • Review medical and social assessments to determine and re-determine consumer eligibility for home care services and the type and amount of service to be provided.
  • Affirm that all approvals are appropriate and completed in a timely manner.
  • Oversee metrics and reporting for the Long-Term Care Web system to ensure the program meets mandated processing timeframes.
  • Utilize reporting outcomes to assist the CASA Director in developing or amending processing workflows.
  • Liaison with stakeholders such as SDOH, vendors, and advocates to foster smooth working relationships.
  • Clarify HCSP practices to external constituents and assist in resolving errors.
  • Work with subordinate staff to quickly and appropriately resolve errors and assist in developing strategies to ensure timely processing.

Requirements

  • A baccalaureate degree from an accredited college and three years of full-time satisfactory experience in social casework, with at least two years in a supervisory capacity.

  • Strong ability to work both independently and collaboratively across diverse teams.
  • Ability to analyze complex problems and provide clear, actionable solutions.
  • Proficiency in Microsoft Excel, Word, and general comfort with technology and digital tools.

Pay

Salary range: $68,475 - $85,590 per year.

Schedule

9:00 AM - 5:00 PM (1 hour flex)
