About the Role

This is a non-exempt, full-time or part-time, hourly role focused on providing direct support within a single program or unit. You will typically support 4-6 individuals in residential settings or up to 10 individuals in periodic programs.

Responsibilities

  • Deliver support per individual service or program plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
  • Assist with daily activities such as personal care, housekeeping, recreational activities, transportation, community integration, shopping, financial management, and citizenship.
  • Provide direct support or assistance in accordance with individual service plans, including socialization, behavioral development, personal care, and recreational activities.
  • Accurately document progress and activity; review records and logs to stay updated on changes in service plans while maintaining confidentiality.
  • Manage individuals’ assets by making cost-effective and appropriate purchases within their budget and according to their preferences.
  • Maintain healthy and professional relationships with individuals, friends, families, guardians, and case managers; implement customer service standards.
  • Report any instance of alleged abuse or neglect according to internal and external standards; report medical, behavioral, and other incidents as required.
  • Respect the rights of individuals served, maintain confidentiality, and assist individuals in exercising their rights.
  • Accompany individuals to medical appointments as needed and relay orders/information to and from medical providers.
  • If assigned, accurately administer or support self-administration of medication, document delivery, and report errors; maintain security of medications and supplies.
  • Monitor individuals’ health, document concerns, communicate with nurses or supervisors, and follow health care directives in emergencies.
  • Follow procedures for medical equipment use, report malfunctions, and ensure proper maintenance.
  • Prepare or support meal preparation according to planned menus and individual dietary needs.
  • Attend orientation, ongoing training, and monthly staff meetings.
  • Work scheduled hours, arrive on time, and notify supervisors of absences; may fill in for other shifts as required.
  • Collaborate with team members, value contributions, and maintain open communication and respect.
  • Comply with workplace safety policies, report hazards, and participate in safety training.
  • Transport individuals safely, ensure proper use of safety equipment, and report accidents or concerns immediately.
  • Perform daily and seasonal housekeeping to maintain a neat, clean, and safe environment; report hazards.
  • Monitor environmental safety, make minor repairs, and report equipment or facilities needing repair.
  • Check water temperature during bathing, participate in safety drills, and protect individuals in emergencies.
  • Perform other duties as required.

Requirements

  • High school diploma or equivalent preferred; six months of experience in human services preferred.
  • Must be at least 18 years of age.
  • Working knowledge of computers.
  • Valid driver’s license in good standing; car registration and insurance if providing transportation.
  • Completion of all state-required training(s) and certification(s) in mandated timeframes.
  • Maintain valid driver’s license, meet insurance requirements, and be approved by management to drive for the company.
  • If driving privileges are lost, employee must sign a Non-Driver form and refrain from driving on behalf of the company until MVR report meets requirements.

Work Environment

Works at the program location and may accompany individuals into the community.

Physical Requirements

Heavy Work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.

Additional Requirements (New Jersey)

  • Must cooperate with licensee and Department of Human Services staff in inspections, inquiries, or investigations.
  • Must have a valid driver’s license in good standing in New Jersey (or state of residence if commuting).
  • High school diploma or equivalent required.
  • Submit to drug testing prior to employment (if hired after 6/15/2018), randomly, and for cause/suspicion.
  • Submit to criminal background checks and be determined not disqualified by the Department of Human Services.
  • Name checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities and the Child Abuse Registry Information (CARI).
  • Successfully complete and demonstrate proficiency in all required trainings.
