Job Responsibilities
- Provides support for individuals and/or groups of persons with disabilities in residential and community settings.
- Implements services and supports to address the person’s goals identified through each person’s Individual Support Plan.
- Aids in the identification and communication of the person’s requests and needs for services and supports.
- Works with an interdisciplinary healthcare team to identify appropriate support methods.
- Assists in the implementation of personal plans, goals, and objectives, facilitating communication between clients and service providers.
- Assists and instructs on food preparation and house cleaning.
- Helps clients identify and acquire valued social roles, as well as healthy personal and social habits.
- Continuously assesses the functional levels for your client, motivating clients and providing consistent reinforcement, assessments, and feedback for all learning activities.
- Dispenses medication, monitors dosages, and makes notes in charts in accordance with your level of training.
- Supports clients at vocational sites to help meet their responsibilities.
- Supports clients during transportation to and from day programs, work sites, and community activities.
Job Requirements
- High School Diploma or G.E.D., or associate degree in a health-related area; or two years’ experience in a health-related field.
- Experience working with individuals with developmental and intellectual disabilities preferred.
- Valid drivers’ license and automotive insurance.
- Flexible schedule and availability to provide on-call coverage as needed.
- Provide CPR after certification.
- CPR, first aid, NCI and/or MedTech certification, a plus.

- Name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities).
- Proof of employment history or references (if required).
- Proof of driver's license, driver's insurance, and vehicle (if required for providing transportation for individuals).
