Caregiver
About the role
Sapphire Health Services specializes in providing comprehensive care and living solutions tailored to meet the diverse needs of seniors and individuals requiring specialized support. We are seeking a dedicated Caregiver to join our team in our Community Based Care Facility. As a Caregiver, you will assist residents with daily activities, promote independence, ensure their safety and well-being, and provide companionship. You will work closely with residents, their families, and the care team to deliver personalized care that meets each individual's unique needs and preferences.
Responsibilities
- Provide assistance with activities of daily living (ADLs).
- Monitor and report any changes in residents' physical or mental health.
- Assist with serving resident meals.
- Provide companionship and build positive relationships with clients to reduce feelings of isolation or loneliness.
- Maintain accurate records of care provided, including daily logs, progress notes, and incident reports.
- Maintain resident confidentiality.
- Encourage clients to participate in activities that promote physical, mental, and emotional well-being.
- Support clients in maintaining independence in their daily routines and decision-making.
- Ensure a safe environment by identifying and addressing potential hazards in community settings.
- Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.
- Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.
- Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.
- Attend staff meetings and training.
- Perform other duties as assigned.
Requirements
- Must be 18 years of age.
- High school diploma or equivalent required.
- Previous experience in caregiving, home care, or community-based care is preferred.
- Ability to work independently and as part of a team.
- Patience, empathy, and a genuine desire to help others.
- Good organizational and time management skills.
- Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.
- Must be able to stand, walk, and reach for extended periods.
- Ability to pass a background check.
