Provide each assigned resident with routine daily care and services in accordance with the resident’s assessment and care plan, as directed by supervisors.
Responsibilities
- Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
- Report all changes in residents’ condition to the Health and Wellness Director as soon as practical.
- Record all entries on notes, SPAs, and incident reports in an informative, descriptive manner.
- Report all accidents and incidents observed on the shift to the Executive Director and Health and Wellness Director.
- Agree not to disclose residents’ protected health information and promptly report suspected or known violations.
- Report any known or suspected unauthorized attempt to access the community’s information system.
- Perform only those care tasks for which you have been trained.
- Ensure the resident’s room is ready (e.g., bed made) and greet residents, escorting them to their room.
- Introduce residents to roommates and other personnel as appropriate.
- Inventory and mark residents’ personal possessions as instructed; store clothing and assist with packing/unpacking during transfers or discharges.
- Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
- Maintain an atmosphere of warmth, personal interest, and positive emphasis, ensuring a calm environment.
- Meet with nursing personnel regularly to identify and correct problem areas or improve services.
- Report all resident complaints and grievances.
- Participate in and receive nursing reports as instructed.
- Follow established policies concerning exposure to blood/body fluids.
- Perform daily care tasks, including:
- Making beds and adding extra covers as requested.
- Checking on residents unable to call for help frequently.
- Answering resident calls promptly and ensuring personal care needs are met.
- Assisting with meals (e.g., identifying food arrangements, providing after-meal care).
- Keeping water pitchers clean, filled, and within reach.
- Assisting with dental/mouth care, bathing, hair care, dressing/undressing, and nail care.
- Shaving male residents and maintaining cleanliness for female residents as instructed.
- Changing wet or soiled gowns, clothing, and linens.
- Positioning bedrest residents correctly and comfortably.
- Assisting with bowel and bladder functions (e.g., bathroom, bedpan, urinal, commode).
- Maintaining intake and output records as instructed.
- Performing restorative and rehabilitative procedures as instructed.
- Providing eye and ear care (e.g., cleaning glasses/hearing aids, installing hearing aids).
- Weighing and measuring residents; recording temperatures, pulse, and respirations (TPRs) as instructed.
- Assisting with end-of-life and post-mortem care as instructed.
- Report injuries of unknown source, including skin tears, and observe disoriented/comatose residents.
- Perform special treatments as instructed and notify the Nurse immediately if a resident is missing.
- Use only equipment you have been trained to use and report defective equipment.
- Maintain confidentiality of all resident care information, including protected health information.
- Report allegations of resident abuse and/or misappropriation of resident property.
- Honor residents’ refusal of treatment and report such requests to supervisors.
- Follow work assignments and schedules to complete assigned tasks.
- Cooperate with inter-departmental and facility personnel to maintain services.
- Notify the facility when late or absent from work.
- Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals per facility policies.
Requirements
- High school education or equivalent (minimum).
- Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to effectively present information and respond to questions from managers and employees.
- Proficiency in applying mathematical concepts (fractions, percentages, ratios, proportions) to practical situations.
- Ability to solve practical problems with limited standardization and interpret instructions in written, oral, diagram, or schedule form.
- Proficiency in Microsoft Suite products.
Physical Demands
- Occasionally lift and/or move up to 25 pounds.
- Prolonged use of desktop or laptop computer.
- Regularly required to sit, stand, walk, talk, read, or hear.
- Frequent use of office-related equipment (copier, scanner, fax, telephone, calculator).
Work Environment
- Low to moderate noise level.

Pay
$16.50–$18 per hour.
