General Summary
Supports the hospital-based Care Transitions team to promote the continuum of care and optimal outcomes for patient populations.
Principal Job Functions
- Commits to the mission, vision, beliefs and consistently demonstrates our core values.
- Assists with post-acute referral processes and communications, including appointments and transportation needs.
- Answers multiple and/or designated telephone lines, prioritizes and routes calls.
- Prepares assigned departmental reports as needed.
- Affords assistance with staff scheduling process.
- Escalates issues to leadership in a timely manner.
- Captures, processes and documents required forms such as Important Message, PASSR’s, travel screening, referrals, ID screen forms and others as deemed appropriate.
- Maintains records accurately, completely, timely and confidentially according to procedures.
- Interacts in an interdisciplinary manner and serves as a resource regarding care coordination initiatives.
- Affords assistance in the identification and reporting of quality issues, contributes to the performance improvement plan and supports quality initiatives.
- Participates in and maintains competencies required for the position and commits to self-directed professional growth, e.g., computer training, continuing education.
- Participates in meetings, committees and department projects as assigned.
- Performs other related projects and duties as assigned.

- Knowledge of computer hardware equipment and software applications relevant to work functions.

Education And Experience
- High School diploma or equivalency required.
- Licensure or certification in a field of medical or allied health area of study preferred.
- Minimum two (2) years of healthcare or equivalent experience preferred.
