Care Transitions Coordinator Home Health

Responsibilities

• Affords patients with guidance through post-acute care processes.
• Utilizes evidence-based clinical guidelines, preventative measures, protocols, and metrics in developing transition plans that prioritize patient-centered care, enhancing quality and efficiency.
• Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and physician groups, as well as executive-level opportunities.

Qualifications

• Graduated from an approved nursing, therapy, or social work program.
• Licensed in the state of practice.
• At least two years of relevant field experience.
• Comprehends federal, state, and local regulations governing Medicare-certified home health and hospice operations.
• Has foundational technology skills, including the ability to operate a mobile device.

Requirements

• Valid state driver license.
• Automobile liability insurance as required by law.
• Dependable transportation in good working condition.
• Safety in operating an automobile under various weather conditions.
