Care Transitions Coordinator Home Health

Responsibilities

Affords patients the opportunity to navigate post-acute care processes, aiming to enhance patient outcomes and improve referral source satisfaction.

Utilizes evidence-based clinical guidelines, preventative strategies, protocols, and relevant metrics to develop comprehensive transition plans that prioritize patient-centered care, ensuring quality and efficiency in post-acute care delivery.

Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, physician groups, and executive-level stakeholders to advance the organization's goals and reputation.

Requirements

• Must be a graduate of an approved nursing, therapy, or social work program.
• Must be licensed in the state where practicing.
• Must have at least two years of demonstrated field experience.
• Must have a thorough understanding of federal, state, and local regulations governing Medicare-certified home health and hospice operations.
• Must possess basic technology skills, including proficiency with mobile devices.

Qualifications

• Education and experience, essential: Graduate of an approved nursing, therapy, or social work program. Licensed in the state of practice. Two years of field experience.
• Education and experience, preferred: Registered nurse or physical therapist. Three years of field experience. Previous experience in home health or healthcare sales.
