Care Team Provider

About the role

We are seeking a mission-driven Care Team Provider to join our dynamic organization. In this role, you will work closely with our primary care providers, primary care RNs and other disciplines to support our primary care patients, ensuring they receive same-day care and have seamless transitions during provider changes.

Responsibilities

• Collaborate with primary care providers (PCP) and nursing staff to support PCHC patients.
• Ensure access to same-day care for patients, addressing their immediate needs.
• Absorb responsibility for managing patients during transitions, such as when a PCP leaves the organization or during provider absences.
• Provide urgent/same day care for unpaneled patients until they are assigned a permanent provider.
• Support desktop management for PCPs to facilitate efficient care delivery.
• Uphold a patient-centered approach to care, prioritizing quality and access.

Qualifications

• Graduation from an accredited MD or DO, Nurse Practitioner and/or Physician Assistant program.
• Current license to practice medicine as MD/DO, a Nurse Practitioner and/or Certified Physician Assistant in the State of Maine.
• Current DEA license.
• Will have up-to-date training and certification in Basic Life Support issued by AmericanHeart Association, American Red Cross, or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
• Minimum of one year of clinical experience is preferred. Experience in ED, acute or urgent care settings is ideal.
