Peak Vista Community Health Centers is a nonprofit health care organization whose mission is to provide exceptional health care to people facing access barriers through clinical programs and education. Pay

$32.39 to $46.96 hourly, based on experience. An additional $2.00 per hour for BSN holders.

$5,000 sign-on bonus paid in installments: $1,250 at 30 days, 90 days, 6 months, and 9 months of employment. Current employees are not eligible.

Loyalty Bonus: $5,000 upon 1 year of service, then $1,250 every additional 3 months worked (prorated for part-time employees).

Schedule

Monday through Saturday, 8:00 AM to 8:30 PM. Rotating 12-hour shifts, 36 hours per week.

Responsibilities

• Assess and triage for the appropriate level of care based on comprehensive risk assessment including diagnosis, clinical judgment, and outcome goals.
• Assess patient’s readiness for change, physical and psychosocial needs.
• Identify and link patients to available resources, social support systems, and family support.
• Assess high-risk and rising-risk patients consistent with current Policies & Procedures for Care Management.
• Facilitate access to quality and cost-effective care.
• Collaborate with and maintain healthy working relationships with community partners.
• Provide education to patients, families, and caregivers.
• Encourage engagement and self-management of patients regarding their own health.
• Provide outreach to individuals and the community.
• Provide in-reach to patients who are presently inactive in their healthcare.
• Utilize technology including Electronic Health Records, state registries, and Health Information Exchange.
• Maintain accurate and complete documentation of all interactions, implementing appropriate protocols for patients’ charts and billing practices.
• Maintain healthy working relationships with team members and staff at all levels of the organization.
• Support the clinical training department and ensure Medical Assistants are competent regarding clinical skills.
• Assist providers with inbox tasks including medication refills (per standing order SOP) and completing paperwork for patients’ external needs.
• Triage and appropriately delegate follow-up needs for patients regarding labs and outside testing.
• Advocate for patients.
• Provide education on disease processes and medication regimens.
• Work with care teams to review schedules and prepare patient charts for visits.
• Understand the immunization schedule and support clinic staff and patients.
• Operate corporate and/or personal vehicles safely and professionally, traveling to any/all Peak Vista’s facilities as needed.

Requirements

• Graduate of an accredited Nursing Program required; BSN preferred.
• Two years' experience in case management and clinical nursing preferred.
• Current Colorado RN License.
• Current CPR (BCLS) certification.
• Valid Colorado driver’s license, proof of insurance, and must meet Peak Vista’s insurance underwriter criteria.
• At least 21 years of age to be an eligible driver on behalf of Peak Vista.
• Spanish bilingual preferred.

Skills

• Basic computer skills: ability to access the intra/internet, manage timecards, review policies, and read company communications.
• Use email to communicate with co-workers, leadership, and other departments.
• Enter, correct, and modify data; format worksheets; and use printing functions.
• Create, run, and print simple PowerPoint presentations.
