About the role

We are seeking a compassionate, team-oriented Care Team Nurse (LPN/RN) to support patients at our Total Health Community Clinic in Monmouth. This role provides practice management support to the clinical team, embracing the Patient Centered Primary Care Home (PCPCH) model by recognizing the patient as the central focus. The Care Team Nurse collaborates with other healthcare professionals to address patients’ wellness, acute illness, chronic disease, disability, and end-of-life needs.

Responsibilities

  • Participate in the ongoing management of patients with stable chronic diseases, as designated by and in consultation with the Primary Care Provider (PCP).
  • Communicate with patients in person and over the telephone regarding management of their chronic disease, coordination of treatment plans/changes, and coordination of services, including follow-up when indicated.
  • Manage the triage, intake, and monitoring of patients with acute medical conditions.
  • Monitor patients in the absence of their PCP, providing necessary nursing intervention when appropriate.
  • Provide patient education in areas related to self-care, such as taking medications appropriately, use of nebulizers and inhalers, diabetes management, wound care, and self-injection, under the direction of the PCP.
  • Explain treatments and procedures to patients.
  • Assist providers with patient examinations, procedures (e.g., minor surgery, casting, nebulizer treatments, spirometry, EKG), and medication administration, as determined by clinical needs and scope of professional nursing practice.
  • Demonstrate cognitive ability to accurately observe, analyze, and interpret general and technical data, both verbal and written, and communicate this information effectively.
  • Recognize abnormal and critical clinical findings and take appropriate action, including notifying the provider as needed.

Requirements

  • Current Oregon RN or LPN license.
  • Current Basic Life Support for Healthcare Providers (BLS) training certificate consistent with AHA Guidelines for CPR/ECC, covering breathing and cardiac emergencies for adult, child, and infant patients.
  • Clinic experience preferred.
  • Ability to exercise independent judgment and decision-making.
  • Ability to work well in a group setting and prioritize competing responsibilities under pressure.
  • Strong problem-solving skills and personal flexibility to perform in an ever-changing work environment with frequent interruptions.
  • Ability to manage high volumes of patients in a short span of time while dealing with unpredictable issues.

Full-Time | Monday – Friday | 8:00 AM – 5:00 PM with optional 4/10 work schedule.
