About the role
We are seeking a Care Team Manager responsible for leading and coordinating an interdisciplinary team (IDT) to deliver comprehensive care to seniors.
Responsibilities
- Managing team operations
- Facilitating team meetings
- Developing and implementing personalized care plans
- Ensuring regulatory compliance
- Overseeing documentation and clinical interventions
- Supervising and supporting staff
- Monitoring participant progress
- Collaborating with healthcare providers and facilities to optimize outcomes
- Participating in quality assurance activities
- Reviewing high-risk cases
- Ensuring adherence to legal and organizational standards
Qualifications
- Minimum 5 years in healthcare, with 3 years in management, preferably in geriatrics
- RN license preferred
- Experience working with the elderly or a willingness to train is required
