At Cedar Ridge Health & Rehab Center in Lebanon, IL, we believe in nurturing nursing talent and empowering our team to grow. We are seeking a compassionate and detail‑oriented Care Plan/Restorative Nurse (LPN or RN) to join our skilled nursing team. In this role you will help promote resident independence, restore function, and improve quality of life through individualized care planning and restorative nursing interventions.
Responsibilities
- Develop and implement individualized restorative nursing programs and resident care plans in collaboration with the interdisciplinary team
- Monitor resident progress and update care plans as needed to meet changing resident needs
- Collaborate with therapy staff to ensure continuity and consistency of care
- Provide hands‑on restorative nursing care focused on mobility, ADLs, and rehabilitative support
- Promote resident independence, dignity, and functional improvement
