About the Opportunity
The Care Navigator is a foundational role within a new direct service practice designed to be human-centered and technology-enabled. This position involves sustained, relationship-based navigation to support transition-age youth and kinship families through their full journey, not just initial contact. The Care Navigator manages complex caseloads, provides ongoing support through transitions and challenges, and contributes to shaping a digital navigation model by leveraging technology and real-time data while centering the lived experiences of those served.
Responsibilities
- Serve as a consistent, trusted contact for transition-age youth and kinship families, building ongoing relationships via various communication platforms.
- Develop and maintain partnerships with community organizations to facilitate warm referrals and collaborative problem-solving.
- Use micro-cash grants strategically to engage families, reduce barriers, and gather data on needs.
- Activate crisis support protocols and coordinate resources as necessary.
- Conduct trauma-informed, culturally responsive needs assessments using digital tools.
- Adapt navigation strategies based on individuals' readiness and feedback.
- Identify systemic gaps and barriers through service interactions and relay findings to inform strategy.
- Collect and document user feedback to improve service delivery.
- Manage active caseloads, monitor progress, and adjust plans over time.
- Co-create personalized action plans and track progress digitally.
- Support individuals through transitions to community partners and programs.
Requirements
- Experience with or personal connection to the child welfare system is valued.
- Strong relationship-building skills and ability to engage families over time.
- Proficiency with technology-enabled service tools.
- Ability to conduct trauma-informed, culturally responsive assessments.
- Capacity to manage complex caseloads and adapt to changing needs.
