They connect health plan members with neighbors who support and motivate each other to stay healthy, focusing on building trust to improve member experience and reduce unnecessary hospital visits. The programs operate in tens of thousands of communities nationwide.

About the role

The Care Navigator is an entry-level role within Care Delivery Operations focused on engaging with patients to complete health assessments and intake appointments. This position plays a key role in identifying factors affecting patients' health and maintaining accurate records, serving as a foundation for career growth into a Care Advocate role.

Responsibilities

  • Conduct health screenings and intake calls with patients using a standard question set.
  • Build rapport quickly to encourage accurate information sharing.
  • Schedule and confirm intake appointments to maintain consistent patient relationships.
  • Identify patient needs through open-ended questions and coordinate next steps.
  • Enter and review patient information in records systems during calls.
  • Adhere to privacy rules including obtaining formal patient consent.
  • Manage daily schedule to meet targets, including maintaining a 60% or higher show rate.

Requirements

  • High school diploma or equivalent required; degree in Human Services, Public Health, or Behavioral Health is a plus.
  • Fluency in English and Spanish or another language is strongly preferred.
  • Comfortable speaking with diverse patients by phone; no prior intake experience required.
  • Familiarity with healthcare, social services, or community outreach is helpful.
  • Proficient with web and app-based systems and adaptable to fast-changing environments.
  • Able to work independently with strong organization and goal management.
  • Willing to complete full training program and pass final assessment with 80% or higher.
  • Must pass drug test and background check prior to starting.

Pay

Hourly pay: $17–$19 per hour, plus performance bonuses.
