Care Navigation Associate at the AIDS Center in Greenwich Village, Mount Sinai Hospital. This role oversees an assigned group of patients, acting as the primary liaison between the patient and the program to implement comprehensive care plans, ensure connection to prevention and treatment services, and promote adherence to the established plan of care.
Responsibilities
- Carries out tasks to execute medical and support service plans, including guiding patients to appointments and accompanying them when necessary.
- Ensures medication adherence issues are addressed.
- Provides education and coaching tailored to specific population needs as required by the program or department.
- Acts as the primary liaison with medical providers to ensure patient adherence (caseload may be up to 20 patient-clients).
- Delivers monthly or weekly health education encounters.
- Performs entitlements reassessment.
- Coordinates logistics for plan adherence, such as appointment reminders, arranging transportation, and making childcare arrangements.
- Documents all patient encounters and enters data into a web-based system; may perform grant-related data entry.
- Performs other tasks as required.
Requirements
- High School diploma or GED with relevant healthcare experience; if no experience, an Associate’s degree is preferred.
- 2+ years of relevant experience is preferred.
Skills
- Excellent written and oral communication skills.
- Spanish speakers are desirable.
- Attention to detail and ability to produce timely, accurate work.
Work Environment
Office environment with occasional accompaniment of patients to appointments. This position is part of the SEIU 1199 collective bargaining unit at Mount Sinai Hospital.
Pay
The salary range for this role is $27.04 - $29.03 per hour. Actual salaries depend on factors such as experience, education, and operational need. This range does not include bonuses, incentive pay, differential pay, or other forms of compensation or benefits.
