Care Manager - Virtual Cardiac Rehab (Contract)

About the role

Movn Health is seeking care managers to join our team dedicated to providing cardiac rehabilitation services. You will manage patients weekly to improve their risk factors and decrease the likelihood of future cardiovascular events. This role offers flexible hours, allowing for a varied career while contributing to Movn's mission.

Responsibilities

  • Guide participants in 1:1 video calls and group education sessions through our 3-month online program.
  • Develop comprehensive treatment plans tailored to individual risk factors and medical needs, including exercise prescriptions, medications, and interventions for smoking cessation, stress, and hypertension management.
  • Engage patients weekly via video, voice, or text messages for coaching, education, emotional support, and modifying treatment plans.
  • Coordinate the approval of treatment plans with Medical Directors remotely.
  • Provide progress reports on an as-needed basis when symptoms are reported or vital signs are not at goal.
  • Share approved treatment plans with referring providers on a monthly basis.
  • Monitor the dashboard for adverse events and address them accordingly.

Requirements

  • Minimum Bachelor’s degree in exercise-related science or nursing.
  • Past experience in telehealth, remote patient monitoring, chronic care management, or virtual care (1-2 years).
  • Certifications required:
    • If Exercise Physiologist: ACSM-CEP preferred or willing to sit for the CEP within 6 months of hire.
    • If Registered Nurse: CCRP certification preferred or willing to sit for the CCRP exam within 6 months of hire.
  • Prior experience in cardiac education, care management, cardiac rehab, and secondary prevention (1-5 years).
  • Tech-savvy, proficient in digital platforms like Slack, Google Meets, Skype, Zoom.
  • Experience in executing quality improvement initiatives to lower re-admissions and improve patient outcomes.
  • A compassionate and empathetic approach to supporting patients.

Qualifications

  • Ability to work 15-20 hours per week, with a commitment to a set schedule for at least 3 months, with flexibility to adjust on a quarterly basis.
  • Opportunity to convert to full-time work.

Skills

  • Bilingualism (Spanish, Mandarin, other languages a plus).
  • Willingness to work flexible hours based on location.
  • Preference for candidates who can work Saturdays.
