Care Manager - Social Worker

About the role

We are seeking experienced Care Managers (MSW or LCSW) to partner with patients, families, and healthcare teams to address social and emotional needs.

Responsibilities

• Conduct psychosocial assessments and develop treatment plans
• Provide therapeutic interventions and crisis intervention
• Coordinate discharge planning and resource referrals
• Collaborate with internal and external interdisciplinary teams to optimize patient care
• Utilize healthcare resources effectively to achieve clinical, financial, and resource outcomes

Requirements

• Master’s Degree in Social Work from an accredited institution
• Washington Clinical Independent Social Worker License
• At least one year of experience in acute medical or mental healthcare settings

Qualifications

Preferred Certifications: ACM/CCM
