Care Manager Rotation Schedule WJMC

Essential Function

Captures written/verbal communication related to the medical treatment plan and communicates it to external agencies as needed.

• Coordinates services across provider lines, between people, and systems to optimize patient outcomes and maintain continuity of care.
• Prioritizes cases based on financial reimbursement and clinical criteria using a daily worklist.
• Works collaboratively with the interdisciplinary team and nursing staff to facilitate transitions in levels of care or discharge planning.
• Establishes rapport with physicians and interacts with them telephonically to discuss the plan of care, documentation, and obtain necessary orders.
• Follows up with patients and caregivers telephonically, evaluating their conditions and providing follow-up based on identified needs.
• Manages pre-certification for post-acute care services and informs physicians, nursing staff, and patients/caregivers about denied services and rights to appeal.
• Maintains a current list of referral resources and refers patients to appropriate inpatient, outpatient, and community resources.
• Keeps updated on insurance coverage and practices to support patients' needs and document IS/SI severity of illness.
• Initializes appropriate post-acute level of care determinations and obtains necessary documentation and data to support discharge plans.
• Utilizes the nursing process to coordinate the care of patients discharged by LCMC and referred emergency department patients.
• Communicates with the nursing staff/care coordinator staff, physicians, patient/caregivers, and other healthcare team members to formulate discharge plans.
• Reviews and discusses discharge instructions with patients/caregivers, including red flag indicators, medications, follow-up appointments, and follow-up care.
• Facilitates the discharge plan while ensuring cost-effective, quality care and managing cases to avoid denials and document discharge delays.
• Coordinates resources needed to ensure continuity of care, such as transportation, medication assistance, home health, durable medical equipment, hospice, palliative care, or social services support.

Experience Qualifications

• Minimum of 2 years of nursing experience in a clinical setting.
• Current LPN license in Louisiana.

License and Certifications

• Current LPN license in Louisiana.

Work Shift

Variable hours (United States of America).
