Care Manager, RN ( Youngstown, OH / PART-TIME / Field-based)

About the Role

FreedomCare is seeking a Case Manager, RN to join our Ohio team. This field-based position requires frequent travel to patients' homes in Youngstown, OH. As a Case Manager, RN, you will conduct initial in-home assessments, orientations for new patients and caregivers, and provide ongoing care management and support.

Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation, ensuring compliance with required forms and procedures.
• Develops, implements, and manages patient activity plans, making revisions and documenting changes as needed.
• Completes client documentation, including written records, reports, and notes, in accordance with agency policy and procedure.
• Coordinates client care through communication and written documentation with physicians, other disciplines, and community resources.
• Provides ongoing coaching to drive effective utilization of best practices, encourages PCA education, and connects families to resources.
• Reviews PCA notes and addresses changes in the consumer’s condition or environment.
• Ensures continuous compliance with regulatory standards, company policies, and procedures.
• Performs other job-related duties as directed by the supervisor.

Qualifications

• Valid and Active Registered Nursing License issued by the Ohio Board of Nursing.
• Ability to articulate cases through effective summaries.
• Ability and flexibility to travel within assigned geographic area; valid driver’s license and car insurance required.
• Proficiency in common software and web-based applications (e.g., Case Management Systems, MS Office) with the ability to teach others.
• Passion for providing high-level customer service, including empathy, active listening, and patience.

Education and Experience

• Diploma from an accredited nursing program. New graduates welcome!
• Preferred: Home care case management and care coordination experience.

Pay

The base pay range for this per diem role is between $40.00 and $45.00 per hour at the commencement of your first year of employment. Compensation decisions are based on factors such as skill set, experience, education, location, and licensure/certifications.
