Care Manager RN Per Diem Variable
About the role
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. Key responsibilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.
Responsibilities
- Assess and plan care for at-risk patients
- Coordinate care and manage resources
- Review and manage utilization of healthcare resources
- Develop and implement discharge plans
- Document interventions and ensure regulatory compliance
- Advocate for patients and their families
Requirements
- Associate's Degree in Nursing or Diploma upon hire
- Certified California Registered Nurse License
- 2 years of acute care experience in a Medical Surgical or Inpatient setting
- 1 year of experience in care management or utilization review in any setting, or successful completion of TIPS program or Case Management Orientation Program
Qualifications
- Bachelor's Degree in Nursing or higher within 3 years of hire preferred
- Case management certification (ACM or CCM) preferred

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $57.28 - $88.92 per hour.
Schedule
This position is Per Diem and works 8-hour, variable shifts.
