Care Manager RN Per Diem Variable

About the role

The RN Care Manager at Providence St. Joseph Hospital Eureka is responsible for carrying out effective case management functions for identified patients. They plan, coordinate, negotiate, procure, and manage the care and needs of complex patients and their families throughout the continuum of care. They communicate daily with medical staff, hospital personnel, and outside resources to ensure appropriate resource utilization according to clinical and financial standards. They monitor the medical record for variances to clinical and financial standards and intervene to achieve the optimum outcome for the patient and the hospital.

Responsibilities

• Carry out effective case management functions for identified patients
• Plan, coordinate, negotiate, procure, and manage the care and needs of complex patients and their families throughout the continuum of care
• Communicate daily with medical staff, hospital personnel, and outside resources to ensure appropriate resource utilization according to clinical and financial standards
• Maintain accurate and up-to-date medical records
• Monitor the medical record for variances to clinical and financial standards and intervene to achieve the optimum outcome for the patient and the hospital

Requirements

• Associate's Degree and Associate's Degree in Nursing or a nursing diploma upon hire
• Certified Case Manager CCM or Accredited Case Manager ACM
• 2 years of acute care experience in a Medical Surgical or Inpatient setting
• 1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire
