Responsibilities

• Provide professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment.
• Assess and plan patient care, coordinate services, and manage resource utilization.
• Conduct discharge planning and ensure regulatory compliance.
• Document interventions and advocate for patients.

Required Qualifications

• Associate’s Degree in Nursing or nursing diploma upon hire.
• Washington Registered Nurse License upon hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.

Preferred Qualifications

• Bachelor’s Degree in Nursing or higher within 3 years of hire.
• National Certification in area of specialty.
• 1 year of experience in care management or utilization review in any setting, or successful completion of TIPS program or Case Management Orientation Program.

Pay

The base pay range for this position is $47.73 – $89.59 per hour. Additional compensation may be available, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. A $10,000 hiring bonus is available for eligible external hires who meet required qualifications and conditions.

Schedule

0.9 FTE night position available, with every other weekend required.
