Care Manager RN

About the role

The External Case Manager performs activities and duties related to preadmission evaluation and admission of patients to services along the rehabilitation continuum of care. Also responsible for relationship building for marketing and patient census building and coordination of care in collaboration with the acute care Case Managers/Social Workers/Discharge Planners to facilitate a smooth transition and most appropriate level of care. The External Case Manager works with appropriate funding sources to establish the most effective manner of managing the human, material and financial resources available for the patient's care.

Responsibilities

• Performs preadmission evaluation and admission of patients to services along the rehabilitation continuum of care.
• Builds relationships for marketing and patient census building.
• Coordinates care in collaboration with the acute care Case Managers/Social Workers/Discharge Planners to facilitate a smooth transition and most appropriate level of care.
• Works with appropriate funding sources to manage the human, material, and financial resources available for the patient's care.

Requirements

• Washington Registered Nurse License.
• 3 years Clinical experience.

Qualifications

• Bachelor's Degree in Nursing.
• CRRN and/or CCM certifications.
• Experience in case management; clinical nursing or utilization management.
• Experience in the rehabilitation of chronically or catastrophically ill or injured individuals.

Pay

Up to $15,000 Hiring Bonus for eligible external candidates who meet all conditions for payment - this is in addition to the fantastic benefits and compensation package offered by Providence that begin on your first day of employment.

Full time.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
