Care Manager RN

Responsibilities
- Conducts in-depth assessments/reassessments covering medical, health, and rehabilitation concerns.
- Performs physical assessments as necessary and interpret clinical findings.
- Certifies level of care determinations.
- Works closely with physicians and other health care professionals.
- Consults with team members and participates in team care management activities.
- Develops care plans, implements and monitors services, consults with other team members, completes case recording and reporting.
- Maintains case load.
- Participates with CCT providing assessments, feedback and planning for patients.
- Aids in the development of organizational programs to create new services.
Required Job Requirements
- Three (3) years experience, two (2) of which are in public health.
- Associate of Science degree in nursing.
- Valid current California license to practice nursing.
PREFERRED
- Geriatrics experience.
- Bachelor’s degree in nursing.

Pay Range
$41.14 - $61.20 /hour
