We are seeking a Care Manager for our team in Oregon. This is a field-based position requiring frequent travel to patients' homes in Portland, OR. Responsibilities include new service plans, quarterly supervisory visits, and renewing service plans.
Program & Position Overview
Structured family caregiving provides daily support based on a patient’s needs while fostering independence in a home environment. The service offers long-term care alternatives for those meeting nursing facility-level care needs, preserving dignity, self-respect, and privacy. The Care Manager is a vital part of our Oregon operations, performing orientations, patient assessments, and caregiver training.
Responsibilities
- Conduct home orientations, including signing of paperwork
- Perform initial assessments during orientations
- Conduct periodic supervisory visits/check-ins for on-care cases
- Train caregivers and patients as required per case
- Interact with caregivers and other stakeholders as needed
- Engage in interdisciplinary meetings with care managers, caregivers, and patients
- Collaborate in care practice and communicate case impressions to appropriate staff
- Critically assess procedures and implement improvements for case evaluation, feedback, and caregiver training
- Wear multiple hats as operations develop
- Travel within the state of Oregon
Ideal Candidate Will Possess
- 2+ years of experience working with frail or elderly disabled adults and caregivers
- Previous field experience in a medical or case management environment, preferred
- Experience with data input, case review/case note entry; Salesforce experience is a plus
- Hands-on experience in home care, preferred
- Working knowledge of ADL/IADL assessments
- Valid driver’s license with an insured vehicle for travel within the state
- Excellent verbal and written communication, organizational, and time management skills
- Strong critical thinking and problem-solving skills
- Proficiency in Microsoft (e.g., Word, Excel, Outlook)

Pay
The base pay range for this role is between $22.00 and $26.00 per hour at the commencement of your first year of employment. Compensation decisions are dependent upon factors including skill set, years of relevant experience, education, location, and licensure/certifications.
