Care Manager MSW
About the Role
Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre-admission to immediate post-discharge.
Required Qualifications
- Master's Degree in Social Work or related field
- Upon hire: Washington Clinical Independent Social Worker Associate License
- 3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination
Preferred Qualifications
- Upon hire: Accredited Case Manager (ACM) OR
- Upon hire: Certified Case Manager (CCM)

Pay
$36.00 - $56.84 per hour. Placement on the wage range is determined based upon relevant job experience and other applicable factors. Additional compensation may be available such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
