Care Manager MSW

About the role

This role involves coordinating care for complex patients from pre-admission to post-discharge, promoting continuity of care, psychosocial outcomes, patient advocacy, and resource management under the supervision of the Care Management Manager.

Responsibilities

• Coordinate care for complex patients throughout the continuum of care (pre-admission to post-discharge)
• Promote continuity of care and psychosocial outcomes
• Advocate for patients and manage resources effectively
• Support a patient-centered, inclusive work environment focused on high-quality healthcare delivery

Qualifications

• Master's Degree in Social Work or related field
• Washington Clinical Independent Social Worker Associate License
• Minimum 3 years of social services experience, including 2 years in acute care or care coordination

Preferred: Accredited Case Manager (ACM) or Certified Case Manager (CCM)

Pay

$36.00 - $56.84 hourly
