Care Manager (Memory Care)

About the role
As a Caregiver, you’ll play an essential role in enriching the lives of seniors, creating meaningful connections, and making a difference every single day. Your role includes providing hands-on care and physical and emotional support to each resident while maintaining a safe and comfortable home-like environment.
Responsibilities
- Responsible for designated group of residents during shift; ensures awareness of whereabouts and conducts frequent checks
- Provide daily personal care support, including bathing, dressing, grooming, toileting, ambulation, transferring, and eating assistance
- Respond to security system and resident call bells promptly
- Help residents maintain independence while promoting dignity, safety, and overall well-being
- Maintain cleanliness and organization of resident rooms and work areas
- Offer companionship, encouragement, and emotional support to residents
- Engage residents in meaningful life skills activities and enrichment programs
- Notify supervisor and/or health care coordinator if a resident has increased care needs
- Other duties as assigned
Requirements
- Certified Nurse’s Aid (CNA) certification preferred
- High School diploma or GED required
- Must be 18 years of age or older
- Prior experience working with seniors preferred
- Strong communication skills with residents, team members, and leadership
- Compassionate, resident-focused approach with superior customer service skills
- Ability to manage multiple priorities effectively
- Sound judgment, problem-solving, and decision-making abilities
