About the Role

The Care Manager I / CCBHC Care Coordinator II will provide case coordination, outreach, and advocacy services to individuals entering CCBHC. This role includes initial screening, referral services, and assisting consumers in connecting to needed resources. Responsibilities may also involve developing psychosocial evaluations, conducting initial health screenings, and creating coordinated treatment plans.

These clinics provide a comprehensive set of services for children and adults, including 24/7 crisis services, outpatient mental health and substance abuse treatment, immediate screenings, risk assessments, diagnoses, and care coordination with emergency rooms, law enforcement, and veteran groups. The CCBHC model aims to deliver integrated care to reduce healthcare costs and improve patient outcomes.

Responsibilities

  • Maintain an active caseload, providing referrals and linkage as needed within areas of expertise and credential limits.
  • Acquire and provide detailed information about assigned consumers to establish the foundation for the Treatment Plan.
  • Work with consumers on a day-to-day basis using professional judgment to implement the team-determined Treatment Plan.
  • Assist in the development, implementation, and revision of individual treatment plans, ensuring services align with the Treatment Plan and monitoring progress toward goals.
  • Communicate consistently with community partners to maintain positive working relationships for existing MOUs.
  • Consult and coordinate with community systems for linkage, referral, crisis management, advocacy, and follow-up to attain treatment goals.
  • Provide crisis management for consumers and make linkages for interventions as appropriate.
  • Deliver individual and group Psychosocial Rehabilitative services.
  • Provide face-to-face skill training and mental health interventions in accordance with the treatment plan.
  • Offer age-appropriate life skills training to consumers individually or in groups.
  • Maintain consumer and program records according to applicable standards, regulations, and grant requirements.
  • Uphold a high level of ethical conduct regarding confidentiality, dual relationships, and professional stature.
  • Demonstrate competent use of Electronic Medical Record (EMR).
  • Participate in continuing education activities to remain knowledgeable in areas of expertise.
  • Attend meetings and meet regularly with a supervisor to exchange pertinent information and receive supervision.
  • Complete CANS and/or ANSA training and maintain ongoing certification.
  • Ensure timely completion of DARMHA Supported Consumer (DSC) paperwork and reassessments within specified deadlines.
  • Participate in required clinical supervision if OBHP qualified.
  • Maintain a flexible schedule in collaboration with a supervisor to meet the needs of consumers and the team.
  • Perform other duties as assigned.

Requirements

Care Manager I

  • High School diploma or equivalent.
  • At least 12 months of paid, full-time employment or 24 months of paid, part-time employment in a directly related adult, child, youth, adolescent, or family services field, including but not limited to:

CCBHC Care Coordinator II

  • Bachelor’s degree in psychology, Social Work, Sociology, Family/Consumer Sciences, Child Development, Substance Abuse, Mental Health, Human Services, Education, Criminology, Counseling, Nursing, Special Education, Family Studies, Marriage and Family Therapy, Recreational Therapy, Music Therapy, Art Therapy, Child and Adolescent Welfare, or Youth, Adult, and Family Services, with a minimum of 1 year of Case Management/Care Management experience (internal or external).
  • Nonspecific bachelor’s degrees with a minimum of 4 years of experience as a Care Coordinator I in a directly related child, youth, adolescent, or family services field.
  • Completion of Case Management training post-college degree.

All Positions

  • Knowledge and skills in community-based behavioral health care and case management.
  • Good interpersonal and communication skills.
  • Agreement to use personal transportation in a work capacity, including transporting consumers.
  • Flexible scheduling is required.
  • Willingness to use reliable personal transportation in a work capacity.

Certificates, Licenses, Registrations

  • Valid driver’s license in accordance with HCI motor vehicle policy.
  • Current Crisis Prevention Intervention (CPI) certification.
  • Current American Red Cross CPR/First Aid certification.

Physical Demands

  • Regularly required to communicate in person or by telephone.
  • Must be able to travel to consumer locations.
