Care Manager I
About the role
The Care Manager I position at IOA is integral to our mission of enhancing the lives of aging adults and adults with disabilities. This role involves conducting comprehensive assessments, developing and monitoring community living plans, and providing support services to ensure high-quality care.
Responsibilities
- Conducts comprehensive assessments and ongoing re-assessments of residents' psychosocial, physical, and mental health, environmental, and spiritual needs.
- Writes comprehensive assessments based on assessment information and develops and initiates a client-centered Community Living Plan.
- Identifies and arranges appropriate community services based on a thorough understanding of Medicare, Medi-Cal, and other entitlement programs.
- Establishes and maintains a care management relationship with clients and their significant others, offering respect, dignity, and support.
- Provides crisis intervention, advocacy, problem-solving, and therapeutic interventions.
- Maintains open communication with community providers, including physicians and other health care and social service workers.
- Educates clients and significant others about resources and provides training when possible.
- Maintains required paperwork and follows a clear, concise, and consistent system of charting for continuity of care.
- Maintains open lines of communication with supervisors and actively participates in team meetings and problem-solving endeavors.
- Participates in research studies and data collection, as required.
- Attends and actively participates in continuous quality improvement initiatives.
- Utilizes supervision appropriately and maintains open lines of communication.
- Applies ethical standards and regulatory/procedural requirements as outlined by the National Association of Social Workers.
- Participates in continuing education classes and in-service training to enhance knowledge and skills.
Requirements
- Bachelor's degree in Social Work or related field required.
- One year of experience working with disabled adults and/or frail elderly required.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and the frail elderly.
- Bilingual highly preferred; English plus Cantonese and/or Mandarin and/or Spanish.
- Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems, and teams of health professionals.
- Demonstrated case management skills and experience in the community health care delivery system.
- Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Computer literacy required.
Qualifications
- Ability to lift up to 25 lbs.
- Ability to reach, bend, and walk.
- Five finger manipulation.
- Speaks and writes English clearly.

Pay
$73,233 - $87,525 annually. This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education, and other factors.
