Position Summary
Under the direction of a Provider and in conjunction with other health care team members, the Care Manager will be accountable for the full adult and pediatric patient care cycle. We are looking for a care manager to enhance the quality of patient management, maximize satisfaction and promote healthy living.
Job Duties
- Assessing, planning, implementing, monitoring and evaluating actions required to meet the patient’s health care needs
- Caregives for safety, timeliness, effectiveness, efficiency, equity, and client-centeredness
- Aid patients in making informed decisions regarding their clinical status and treatment options
- Incorporate current patient literature regarding effective engagement, communication, care management and behavior change strategies and clinical practice
- Implement evidence-based care, chronic disease protocols and guidelines
- Identify improvement opportunities in patient care and coordinate resolution protocols
- Coordinate patient care by linking patients to internal and external community resources
- Collaborate with the primary care provider, patient, and members of the health care team to assess, develop and implement a patient care plan
Minimum Requirements
- Knowledge equivalent to that which normally would be acquired by completion of a four-year college degree program in a Social Work; one to three years of related and progressively more responsible or expansive work experience in care management including chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education; and/or experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical
- Knowledge of electronic medical records
- Or an equivalent combination of education and experience
- Knowledgeable in healthcare management systems and Microsoft Office Word and Excel
- Excellent written and verbal communication skills
- Strong organizational skills
- Ability to travel locally
Desired Qualifications
- Bachelor's degree in social sciences, social work, nursing, or a related field
- At least 2 years of experience in public health or a related field
- At least 1 year of experience in case management
- Proficient in Healthcare Management Systems and Microsoft Office Word and Excel
- Excellent written and verbal communication skills
- Strong organizational skills
- Ability to travel locally

Work Hours
Work hours are typically 8:00 AM - 5:00 PM but may vary as necessary to accommodate clinic operations.
