About the role
Care Managers provide outreach and enrollment services to children presumed to meet eligibility requirements of New York State's Children's Health Home program. They gather assessments, enter documentation confirming eligibility, and identify areas requiring supportive services. They work with children, families, and service providers to create a comprehensive Plan of Health Care, coordinate ongoing involvement, and facilitate transitions between health homes.
Responsibilities
- Gather assessments and enter documentation confirming eligibility and identifying supportive services needs
- Create and coordinate comprehensive Plans of Health Care
- Identify and connect families with additional service providers as needed
- Document progress and arrange for transitions between health homes or to other levels of service
- Work effectively in team settings
- Communicate with families and service providers in a clear and professional manner
- Work with a caseload of 1 to 20 individuals
- Coordinate services across various service providers and agencies
- Respond to changing priorities in a fast-paced environment
Requirements
- Associates Degree required; Bachelor's Degree preferred
- 2 years relevant experience in child welfare, developmental disabilities, behavioral health, primary health care, or social services
- Experience coordinating and participating in team settings
- Solid writing and verbal communication skills
- Ability to work effectively and knowledgeably across diverse cultural, ethnic, and racial communities
- Valid NYS Driver's License required with a clean MVR

- Ability to work evenings and/or weekends
- Able to lift and carry up to 25 pounds with or without reasonable accommodation
- Willingness to respond to the needs of a culturally diverse population

- Professional and courteous demeanor with internal and external constituents
Skills
- Strong written and verbal communication skills
- Ability to work in a trauma-informed manner
- Knowledge of the Children's Health Home program and eligibility criteria
- Experience working with families and service providers in child welfare, developmental disabilities, behavioral health, primary health care, or social services
Pay
$20.00 - $21.00 per hour based on education and experience.
Schedule
Available to work evenings and/or weekends.
