Care Manager (CNA/HHA)

Job Summary The Care Manager supports residents in their daily activities, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities, following individualized care plans. They provide direct care such as bathing, dressing, toileting, and feeding, ensuring residents' safety, comfort, dignity, and confidentiality. Responsibilities include escorting residents to meals and activities, maintaining their grooming and safety, documenting services, and reporting any condition changes to supervisors. Qualifications required are certification as a Nursing Assistant or Home Health Aide in New Jersey, with preferred experience working with the elderly or those with dementia. Physical abilities include standing, walking, lifting up to 50 lbs, and using proper body mechanics.
