Care Manager BSW

Role Overview The Inpatient Bachelor of Social Work (BSW) Care Manager provides comprehensive, patient-centered care management services in an acute care environment. Responsibilities include care coordination, discharge planning, documenting interventions, ensuring regulatory compliance, and advocating for patients. The primary goal is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive, behavioral, or legal barriers to access resources. Qualifications & Skills Bachelor's Degree in Social Work from an accredited program.At least 1 year of social work experience in healthcare settings. Preferred Qualifications Experience in acute care or completion of TIPs or Case Management Orientation Program.Case management experience and basic understanding of medical terminology. Key Duties Coordinate patient care and discharge planning.Document interventions and ensure compliance with regulations.Advocate for patients and identify resource needs to overcome barriers.
