Care Manager

About the role

Coordinate and manage care throughout the patient's acute inpatient illness, ensuring continuity of services and appropriate utilization and management of resources. Demonstrate leadership abilities by exhibiting a proactive approach to the care management process.

Responsibilities

• Plan, facilitate, advocate, educate, monitor, evaluate, and measure outcomes for each identified patient population as part of the case management process.
• Demonstrate commitment to group goals and teamwork.
• Assume responsibility and accountability for own duties and performance.
• Participate in the health system's quality improvement initiatives, including facilitation and implementation of disease management processes, performance improvement, quality, and outcome management.

Requirements

• Valid Louisiana nursing license (RN or LPN).
• Minimum of 5 years clinical experience in an acute care setting; management experience preferred.
• Utilization and/or Case Management experience desirable.
• Prior experience or knowledge of severity of illness/intensity of service criteria system, utilization, or medical necessity/certification review preferred.

Schedule

Full-Time, Day shift (08:00-16:30).
