Key Responsibilities
- Conduct home visits to assess and create personalized care plans.
- Train and support caregivers, ensuring they can meet their loved ones' needs.
- Monitor and report on clients’ health, coordinating with their primary care physicians, community resources, and more.
- Facilitate smooth transitions and ongoing care for clients.
- Maintain compliance with all relevant regulations and standards.
Qualifications
- Bachelor’s degree required; preferably in social work, human services, or a related field.
- LSW or LISW in state of employment preferred.
- At least one (1) year of experience working with elders or adults with disabilities.
- Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
- Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
- Training, evaluating, and supervising caregivers.
- Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting their care related needs.
- Documenting in an accurate and timely manner; maintain a clinical record for each morbidity patients.
- Monitoring the health status of all members to ensure that all needed care is properly delivered.
- Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc).
