About the Role

Penick Village is seeking a compassionate, organized, and experienced Care Manager to lead and coordinate high-quality Home Care services. This role is ideal for someone who is passionate about helping seniors remain safe and independent in their homes while providing exceptional support to families and caregivers. The Care Manager serves as the primary liaison between clients, families, caregivers, healthcare providers, and the Home Care team to ensure person-centered, coordinated, and compassionate care.

Care Management & Client Support

  • Conduct comprehensive client assessments, including physical, cognitive, psychosocial, environmental, and safety needs.
  • Develop, implement, and regularly update individualized care plans.
  • Evaluate home safety and recommend services, equipment, or interventions to support independent living.
  • Monitor medication management and identify compliance concerns.
  • Recognize changes in client condition and respond appropriately to urgent or emerging care needs.
  • Coordinate care with physicians, nurses, therapists, hospitals, hospice providers, and other healthcare professionals.
  • Support smooth transitions between home, hospital, rehabilitation, assisted living, and skilled nursing settings.
  • Assist clients and families with advance care planning and navigation of community resources.
  • Assist with long-term care insurance documentation and claims.
  • Serve as a trusted resource for clients and families by providing education, advocacy, and ongoing communication.
  • Promote client dignity, independence, and quality of life through person-centered care.

Staff Leadership & Supervision

  • Supervise home care caregivers to ensure exceptional quality of care and customer service.
  • Conduct supervisory visits, client satisfaction visits, and caregiver observations.
  • Participate in recruiting, interviewing, onboarding, scheduling, and performance evaluations.
  • Provide coaching, mentoring, education, and professional development for caregivers.
  • Ensure compliance with infection prevention, safety practices, and agency policies.
  • Address client and caregiver concerns promptly and professionally while promoting positive outcomes.

Operations & Administrative Responsibilities

  • Oversee implementation of services outlined in individual service agreements.
  • Coordinate scheduling and staffing coverage to meet client needs.
  • Maintain accurate, timely, and compliant documentation in accordance with agency policies and state regulations.
  • Participate in quality assurance, audits, and performance improvement initiatives.
  • Assist with incident reporting, investigations, and follow-up documentation.
  • Participate in state surveys and assist with responses to findings or corrective actions.
  • Promote Penick Village Home Care services through community outreach and professional networking.
  • Collaborate with the interdisciplinary healthcare team to ensure continuity of care.
  • Participate in rotating on-call coverage, including evenings, weekends, and holidays.

Preferred Education

  • Master of Social Work (MSW) or Registered Nurse (RN) preferred.
  • Bachelor of Social Work (BSW) or Licensed Practical Nurse (LPN) with relevant experience will be considered.

Preferred Experience

  • Certified Geriatric Care Manager preferred.
  • Three to five years of experience in geriatrics, home care, care management, case management, or senior services.
  • Experience working with older adults, including individuals living with dementia and chronic medical conditions.
  • Knowledge of long-term care insurance and community resources is a plus.

Requirements

  • Current North Carolina RN, MSW, or BSW licensure/certification as applicable.
  • Valid North Carolina driver's license with a satisfactory driving record.
  • Reliable transportation for client visits.
  • Ability to travel throughout the service area.
  • Ability to participate in an on-call rotation.
  • Ability to meet all pre-employment requirements, including background check, drug screening, and health screening.
