Care Manager

About the role

As a Care Manager at Lawndale Christian Health Center, you will be responsible for coordinating care between patients, their families, and healthcare providers to ensure optimal health outcomes.

Responsibilities

• Assess patient needs and develop care plans
• Coordinate care among various healthcare professionals
• Provide education and support to patients and families

Requirements

• Bachelor’s degree in Healthcare Administration, Nursing, Social Work, or related field
• At least 2 years of relevant experience in healthcare management or patient care
• Excellent communication and interpersonal skills
• Proficiency in Microsoft Office Suite
