Care Manager

About the role

Harmonia Collaborative Care is seeking a full-time Health Homes Case Manager to support adults in Erie County who are navigating behavioral health needs, chronic medical conditions, and barriers to care.

Responsibilities

• Manage a caseload of approximately 45–50 adults enrolled in Medicaid Health Home services
• Complete intake, screening, eligibility reviews, assessments, and reassessments
• Identify client strengths, needs, barriers to care, service gaps, and available supports
• Develop individualized care plans with clear short- and long-term goals
• Care coordination with behavioral health providers, primary care, specialists, and community agencies
• Connect clients with housing, benefits, food resources, legal supports, financial assistance, and other community services
• Maintain accurate, timely electronic case records in accordance with NYS Health Homes and Medicaid standards

Requirements

• Bachelor’s degree in psychology, Social Work, Human Services, or a related field
• Two to three years of relevant community-based experience in behavioral health, care management, case management, or social services
• Knowledge of behavioral health, chronic health conditions, community resources, and care coordination
• Valid NYS driver’s license, reliable vehicle, and good driving record

Qualifications

• Full-time position
• Based in Derby, with home visits and community travel throughout Erie County
• Primarily office-based with some work-from-home flexibility
• Some evening hours may be required
• No weekends
